Life Care Services Australia

Older individuals with chronic illnesses and disabilities may struggle with daily tasks, making it challenging for them to independently care for themselves. As a result, many seniors seek assistance from outside sources, such as moving into assisted living communities or nursing homes. A community participation project aims to engage individuals in community initiatives and provide them with opportunities for involvement that can enhance their health and well-being. As a crucial member of the NDIS team, a support coordinator works with you to create, organize, and assist with your NDIS plans. The support coordinator acts as a liaison between you and your service providers, ensuring that communication is effective and that your needs are being met.
