Post-Hospital Care at a glance
What's included
Nursing, personal care, and therapy at home after hospital discharge or surgery.
On FindMyHelp you can compare providers who describe how they deliver post-hospital care, their service area, and what a typical visit or engagement includes.
Funding and how to pay
Clinical nursing at home may be funded through NDIS core or capacity building supports, aged care packages, or private arrangements depending on your needs and eligibility.
NDIS funding depends on your plan and whether the support is reasonable and necessary for your disability-related needs. Self-managed and plan-managed participants can often use registered or unregistered providers; NDIA-managed plans generally require NDIS-registered providers.
Aged care supports may be available through Support at Home or the Commonwealth Home Support Programme (CHSP) for eligible clients. An assessment through My Aged Care determines what you can access.
How to compare providers
When comparing post-hospital care providers:
- Ask about experience relevant to your situation and funding type
- Check service area, wait times, and staff continuity
- Confirm how they invoice NDIS, Support at Home, or private clients
- Compare how clearly they explain what is included in their service
