In 2025, ACE Supports, together with the South East Queensland Transitional Care Alliance, presented a transitional care model to Queensland Health and the Department of Families, Seniors and Disability Services. This framework highlighted the practical resourcing needed to support safe, cost-effective hospital discharges for younger patients under 65.
