Enable Horizon Disability Support Services is a registered NDIS provider supporting participants across Perth and surrounding areas. We provide person-centred, respectful and reliable support tailored to each participant’s goals, preferences and individual needs.
Our approved services include Community Nursing Care, Personal Activities, Travel and Transport, Daily Tasks/Shared Living supports, Community Participation, Life Skills Development, Household Tasks, Employment Support, Life Stage and Transition Support, and Group/Centre-Based Activities.
We also support participants living with psychosocial disability and participants who may require more complex or coordinated support, where the required supports fall within our approved registration groups and staff competencies.
We welcome NDIA-managed, plan-managed and self-managed participants and currently have capacity for new referrals. Families, support coordinators and recovery coaches are welcome to contact us to discuss individual support needs.
